Support Coordination

An NDIS plan is a document. Services are what change your life. Support coordination is the work of getting from one to the other — understanding what your plan actually funds, finding providers who have capacity, getting agreements in place, and stepping in when something stalls.

Motherly Health Care is a registered NDIS provider (registration number 4053350558) delivering Level 1 and Level 2 support coordination across Oakville, Windsor, Richmond, Pitt Town, Box Hill, Rouse Hill, Kellyville, Blacktown and the wider Hawkesbury and north-west Sydney region, as well as metropolitan Adelaide.

The levels of support coordination

Level 1 — Support Connection

Short-term help to understand your plan and connect with providers, informal supports and community services. The aim is to get you confidently using your plan on your own. This suits people whose situation is reasonably stable and who mainly need a hand getting started.

Level 2 — Support Coordination

Ongoing coordination across multiple providers and services. This is the level most people mean when they say support coordination. It covers building your capacity to direct your own supports, resolving problems when services break down, and making sure the parts of your plan work together rather than against each other.

A third level, Specialist Support Coordination, exists for participants in complex situations that need a coordinator with specific clinical or risk-management expertise. If that is what your plan funds and what you need, we will tell you honestly whether it is within our scope or whether you would be better served elsewhere.

What a support coordinator actually does

Making the most of your allocated funding

Plans are finite. A significant part of coordination is helping you spend the budget you have in a way that gets you the most useful support — not exhausting a category in eight months, not leaving funding unused at plan end because nobody got around to setting a service up. We will tell you when spending looks off-track, early enough for it to matter.

We will also be straight with you about what your plan does not fund. If something you want is not reasonable and necessary under the scheme, or sits with the health or education system instead, hearing that early is more useful than a polite maybe.

Choice and control, in practice

You choose your providers. A support coordinator’s job is to give you real options and the information to compare them — not to route you toward services we happen to deliver. If you would like us to coordinate your plan and also provide your daily supports, that is your call to make, and we will put the potential conflict of interest in writing so the choice is genuinely informed.

You can change coordinators. If ours is not working for you, you are entitled to move, and we will hand over cleanly.

How support coordination is funded

Support coordination is funded from the Capacity Building budget, under Support Coordination. It is a stated support, which means the funding is allocated specifically for it and cannot be moved to other categories. If you cannot see it in your plan, you may not currently have it funded — and that is worth raising at your next plan reassessment if you think you need it.

Every plan is different. If you are not sure which budget a support sits under, bring your plan to the conversation and we will go through it with you line by line — or talk to your support coordinator or Local Area Coordinator before you commit to anything.

Bring us your plan — even if you have had it for months and have not started using it. That is a very common place to be.
